Essential to minimizing the consequences of heart disease is to understand its causes, manifestations, and treatments.

Education is a cornerstone of the cardiac care we provide for our patients and their families. We give patients the information they need in order to make informed decisions with us about their plan of care. Instruction can consist of one-on-one patient teaching, participation in self-help programs, written handouts for specific diseases and conditions, or exploration of the vast resources of our Patient and Family Learning Center.

Whether a patient, or the family member of a patient with heart disease, we hope the information in this section will take some of the mystery out of the condition, our medical center, and the process by which we diagnose and treat cardiac conditions.

What to bring at your first Appointment?

Bring copies of all relevant and recent cardiac records, including reports of any recent procedures, such as ECG, echocardiogram, stress tests, Nuclear Cardiology tests, cardiac catheterizations and angioplasties, etc. Bring latest blood test reports, including fasting cholesterol and lipids, if available.
​ Bring a list of all your medications, with dose and frequency. Or, bring all medication bottles themselves! Bring a list of all medications to which you are allergic.
If you are covered by POS, EPO or HMO insurance that requires a valid referral be sure to bring valid authorization the day of your visit.
​ All co-payments and deductibles are due at the time of service and can be paid by cash, check or credit card.
Please bring all current insurance cards to your visit so the office can make a copy for your file.
